Treatment Consent
By receiving services, I acknowledge that I have voluntarily chosen to participate in facial massage, skincare, waxing, brow services, or other esthetic treatments provided by the practitioner.
I understand that results vary from person to person and that no specific outcome or result can be guaranteed. I understand that temporary redness, sensitivity, tenderness, swelling, bruising, breakouts, skin irritation, or other minor reactions may occur following treatment.
Health Disclosure
I agree to inform my practitioner of any medical conditions, allergies, medications, recent cosmetic procedures, injuries, infections, skin sensitivities, pregnancy, or any other condition that may affect my treatment.
I understand that withholding relevant health information may increase the risk of adverse reactions and may limit the practitioner's ability to provide services safely.
Contraindications
I understand that certain conditions may require modification, postponement, or cancellation of treatment, including but not limited to active infections, contagious skin conditions, open wounds, recent cosmetic injections, recent facial surgery, severe acne flare-ups, or other conditions deemed unsuitable by the practitioner.
Liability Release
I acknowledge that I am receiving services at my own risk and release the practitioner, business, employees, contractors, and affiliates from liability for any injury, allergic reaction, irritation, or adverse outcome that may occur, except in cases of gross negligence or willful misconduct as permitted by law.
I understand that I may stop or modify treatment at any time and agree to immediately communicate any discomfort during the service.
No Medical Claims
I understand that esthetic and facial massage services are not medical treatments and are not intended to diagnose, treat, cure, or prevent any disease or medical condition. Any information provided during services is for educational purposes only and should not be considered medical advice.
Photography Release (Optional)
I authorize the use of before-and-after photographs and video for educational and marketing purposes only with my written consent.
Cancellation Policy
Appointments canceled with less than 24 hours' notice may be subject to a cancellation fee of up to 50% of the scheduled service price.
No-shows will be charged up to 100% of the scheduled service price.
Refund Policy
Due to the time reserved, professional services rendered, and the individualized nature of treatments, all services are final and non-refundable.
If concerns arise regarding a treatment, clients must notify the business within 48 hours of their appointment so that we may evaluate the situation and, when appropriate, offer reasonable corrective measures at our discretion.
Products purchased for home use may only be returned if unopened and in original condition within 14 days of purchase unless otherwise prohibited by manufacturer policy.
Acknowledgment
By booking and receiving services, I acknowledge that I have read, understood, and agreed to this consent, liability waiver, cancellation policy, and refund policy.
